ERX PAPERWORK EXPLAINED
The following document assists with explaining the paperwork that Emergency Restoration Xperts (ERX) might
use during the life of a claim. Please understand that not every project will see all the following documents. If
there is a question about a document that is received, please refer to this page before reaching out to your
ERX Representative. Thank you!
- Anti-Microbial Right to Know- This is an informational document that explains what is in the chemical we
spray and what items we use it on. This is attached in the Welcome Email Brochure. - Change Order- If anything changes in the scope of work that hasn’t been identified in the approved
estimate, it will need to be listed in this document and signed by the Policyholder. These can also be used
to list credits due and change materials from what was originally selected. Used during the Reconstruction
phase, this agreement is between the Policyholder and ERX. Once approved, a 50% invoice will be sent to
the Policyholder and will need to be paid before the listed work begins. The remaining 50% will need to be
paid once the listed work is completed or has otherwise specified by the Project Manager. - Certificate of Satisfaction (COS)- There are multiple times that these documents can be signed
throughout the life of a project (usually at the end of Mitigation and Reconstruction). The COS states that
ERX has completed a certain phase of the project and can be sent to the insurance company to update
them on the current status. Failure to sign any of the COS’s can result in the delay of payment and
ultimately the entire project. - Construction Services Contract – Before the reconstruction phase can begin, this form must be filled out
completely. This is a binding contract, fully detailed for the customers’ protection as well as ERX’s. If you
would like a copy of this contract before the reconstruction phase (to review) please ask your ERX
representative. - Customer Selection Form (Product Selection Form)- In this form, a list of all materials that will be used
on a project will need to be written and signed off by the policyholder. Failure to make the list of selections
in the allotted time can result in project delay. Used on all Reconstruction projects. Any changes after
selections are made will be listed in a Change Order (see above). - Draw Schedule (inside Construction Services Contract)- Once an Estimate has been approved, a draw
schedule will be built. This explains when payments need to be made and for how much. Typically, this is
centered around key progress points that are determined by the Project Manager such as drywall being
completed, flooring installed, etc. This is for Reconstruction projects only and will need to be signed before
the start date. - EPA Form- This pamphlet explains lead. What it is, what to look for, what its effects are, and how to take
care of it. You will find this information attached in the Welcome Email Brochure. - Estimate- Each phase of the project will have an estimate sent to the insurance carrier for approval,
unless otherwise specified by the Project Manager. Some of the different phases are Roofing, Mitigation,
Reconstruction, Abatement, and Contents. Not all projects are going to have every phase listed above.
These estimates will have detailed line items of all work being performed to complete the phase being
worked on. Every line item is needed to complete the project and the total amount will need to be collected
at the end of each phase before continuing. - Invoice- When payments are due, ERX’s Accounting Department will send an invoice to the policyholders’
email address. Once an invoice is received, please reach out to your Project Manager for any questions
you may have about it. Remember, delays in paying an invoice can delay the project’s completion. All
phases receive invoices at different times. - Project Workflow- A simple schedule, written out for the customer to know where their project status
currently is and what will be coming next. - Supplement- This is a request for additional funds and typically arises when unforeseen issues or
changes occur during the course of a project that weren’t accounted for in the original estimate. These
could include discoveries of hidden damage, code requirements that were overlooked, or necessary
changes in materials or labor costs. - Waiver of Liability- If anything is requested to be done by the policyholder and it contradicts the standard
procedures of ERX then this document will need to be signed. Depending on the request, this can also
void ERX’s warranty if the integrity of work is compromised. This can be used at any phase of the project. - Waiver of Warranty- States that ERX is not responsible for contents items that are not moved by their
contents department. If the policyholder decides to move contents on their own or let another company
perform the packout, the policyholder assumes all risk and liability. - Work Authorization- Before any work can commence, (including the setup of equipment) this document
will need to be signed by the policyholder. It allows us to perform work on the property and once work
begins, it makes the policyholder responsible for any payment to the contractor. Don’t panic! We will work
with you and your insurance to get charges covered by the carrier. Payments for the work can be sent from
the carrier directly to the Policyholder. This guarantees that the payment will be handed over to ERX at the
completion of the project or at the agreed upon time. All phases will need this document and if you have
